Our staff diligently works around the clock to meet the needs of our patients and a Registered Nurse is always on call 24 hours a day, 7 days a week. Our system creates a constant flow of communication so we can identify any potential issues that could require hospitalization to be caught at its onset and be treated at home. This in turn ensures client/family/caregiver comfort and medical costs down. By following this process, Trinity avoids unnecessary institutionalization.

Trinity Health Care Services takes great pride in educating the client/caregiver or client family in the following areas:

• Ensure patient/family/caregiver involvement.
• Remind patients and families that it is important that they have open communication with the health care team.
• Discuss the plan of care and address any concerns that the patient may have.
• Make sure to discuss the patient’s likes, dislikes and expectations so the home health team can work effectively in the comfort of your home.
• Ask for contact numbers so if you experience any issues, you know how to reach your health care provider.

We also take pride in our staff. Skilled nurses working with Trinity must be graduated from a licensed and accredited school of nursing and holding a current and active nursing license in the state of Florida. Trinity seeks to provide the highest quality of training to all its nurses to ensure the safety, comfort, and maximum care of its patients at all times. Therefore, in accord with the policies of the Florida Board of Nursing and of Trinity Health Care Services, LLC., all RNs and LPNs must complete 24 hours of appropriate continuing education during each renewal period.

Continuing education courses are provided at the Trinity office; the courses include but not limited to, HIV/AIDS/OSHA (Occupational Safety and Health Administration), Domestic Violence, Prevention of Medical Errors, and HIPAA (Health Insurance Portability and Accountability Act). Annual mandatory in-services are also held for the skilled nursing staff regarding Emergency Management Operations, Fall Prevention (adult and child), Medication Management, Communication and Documentation, Complaints-Resolution (Grievance) and Incident Reporting, and Coumadin/Warfarin Therapy. All mandatory in-services required by each contracted provider differ according to the various contracted providers. Examples include the Waiver Program that requires a core assurance certificate.
