Trinity Health Care Services, Inc. provides the following services:

Skilled Nursing
A Registered Nurse or a Licensed Practical Nurse provides this level of care.
However, a licensed physician must require this kind of service.

If the payer source is Medicaid, the patient's age play an important role in thedelivery of this level of care.

a) Patient < 21 years old (under)
are entitled to visit or 24 hours of Private Duty Nursing (PDN),
Respiratory Therapy, Occupational Therapy, Physical Therapy, Speech Therapy etc…

b) Patient > 21 years old (over)
are only entitled to visit with a maximum of four (4) visit per day.

Personal Care
CNA/HHA, PCA, or HHA provides this service to patient under the supervision of an RN. This kind of service is related to visit (one (1) to four (4) times a day or any combination with skills nurse, with a maximum of four (4) visits per day.

Respite Care, Homemakers, Companion & Sitters, Live-in/Live-out
This service can be provided by CNA, HHA, PCA and can be requested by patient/patient's family or others.

Managed Care
Assistance with Placement is a courtesy from Trinity to help you find the right place for your love ones to spend the rest of their life.
Vital obligations prevent you to dedicate an amount of time to manage such care for your loves ones. A numerous factors constituted a road block to express your gratitude or your love toward their basic needs. Such as distance or lack of clinical knowledge. Trinity may assures that responsibility for you.
